What you are describing above is helpful for understanding, but it's identical (except for the omission of Level A) to the CD-XA standards. Decoding for that audio format most certainly exists out there, see e.g. MESS, FFmpeg, and many other sources (I could provide sample code but it is already out...

There are no differences in the actual data bits between the EU and US versions.
The only difference is in the packaging and the mastering (leadin and leadout areas).

Check out this article from February 2017: The 7th Guest CD-i DVC Compatibility / Cake Puzzle Bug.

It's about the difference between the V1 and V2 versions and even offers a patch to change a V1 disc image into V2.
I haven't been able to get it working yet, though...

You can, if you "rewind" the date of your PC to before 2012/01/01 or use a tool so that only CD-i Emulator sees such a date.

I used the version of CdiEmu that has the ROMS bundled in the ZIP file that's floating around the net.
